Help Make Kankakee County Healthier

The Partnership for a Healthy Community wants to hear from you. Take the 2026 Kankakee County Community Health Survey and share what our county needs to be healthier. It covers physical and mental health, quality of life, access to services, and more.

The survey is anonymous and takes less than 10 minutes. You must be 18 or older to take it. For other languages or a paper copy, email survey@metopio.com.

Riverside Medical Center partners with the Kankakee County Health Department, St. Mary's Hospital, The United Way of Kankakee and Iroquois Counties, and representatives from multiple other service providers within the community to provide leadership, staffing, and funding for the Partnership for a Healthy Community. For this most recent Community Health Needs Assessment (CHNA), this group began its work in 2023 and continued through 2024 to develop a refreshed plan for 2025-2027.

The Partnership for a Healthy Community contracted with Metopio to facilitate the MAPP 2.0 assessments to identify strategic priorities and focus areas. The purpose of the Implementation Plan is to articulate Riverside Medical Center’s specific support for these community-identified priorities.
