Transfer Your Medical Records
Your health history matters. We want to make sure your new Riverside provider has everything they need to give you the best possible care. Here's how to get your records transferred.
Step 1: Request Your Records
Contact the medical records department at your previous healthcare provider. Ask them to send your records to your new Riverside provider's office. Most facilities can send records by fax or through a secure electronic transfer.
Step 2: Bring What You Have
If you have copies of recent lab results, imaging discs, medication lists, or discharge summaries, bring them to your first Riverside appointment. Every detail helps your new care team.
Step 3: We'll Take it From Here
Your Riverside provider will review your records and work with you to build a care plan that picks up right where you left off. No starting over.
Currently in active treatment? If you're managing an ongoing condition or in the middle of a treatment plan, please let us know when you schedule your first appointment. We'll prioritize getting your records and coordinating your care so there's no gap in treatment.

You can browse our provider directory and filter by specialty, location, insurance, and whether the provider is accepting new patients. If you'd like a personal recommendation, call our scheduling team at (815) 933-1671 and we'll help match you with a provider who fits your needs.
How do I get my medical records transferred to Riverside?
Contact the medical records department at your previous healthcare provider and ask them to send your records to your new Riverside provider's office. Does Riverside accept my insurance?

What if I'm currently being treated for a condition?
If you're in active treatment or managing an ongoing condition, please let us know when you schedule your first appointment. We'll work to get your records quickly and coordinate with your new Riverside provider so your care plan continues without interruption.
Does Riverside have an emergency room?
Yes. The Riverside Medical Center Emergency Department is open 24 hours a day, 7 days a week, and is designated as a Level II Trauma Center. If you are experiencing a medical emergency, call 911 immediately.
What if I need care right away but it's not an emergency?
Our Immediate Care locations offer walk-in visits for non-emergency needs like minor illnesses, injuries, and more. No appointment is needed. You can also use our Get Care Now guide to help you decide where to go based on your symptoms.
Can I keep using MyChart?
Yes. If you already have a MyChart account from another healthcare facility that uses Epic, you may be able to link it to Riverside Healthcare. Log in to your existing account and search for "Riverside Healthcare" to connect. If you're new to MyChart, you can sign up for free online or at your first visit.
Does Riverside offer behavioral health services?
Yes. Riverside offers a range of behavioral health services including outpatient counseling, psychiatric care, substance use treatment, and specialized programs for children, adolescents, and adults. Does Riverside offer rehabilitation services?
Yes. Riverside provides both inpatient and outpatient rehabilitation services, including physical therapy, occupational therapy, speech therapy, and home health.
