Physician Services performs credentialing for Riverside Medical Center, Riverside Ambulatory Surgery Center and Oak Surgical Institute. In addition, it manages the medical staff organization, including bylaws, policies and procedures and meetings of the divisions, service lines and committees.

Provider Affiliation Verification Requests

We are pleased to announce that Riverside Medical Center verifies practitioner affiliations online. Due to the number of responses we receive, specific hospital request forms will no longer be completed. In addition, the Medical Staff Office will no longer process requests for the completion of evaluation forms by the practitioner's chairperson. If such an evaluation is necessary, please contact the practitioner to obtain it from their Department Chairperson.
